Accessing Your Patient Record

You have the right to withdraw your consent for the different ways we use or disclose your personal health information by contacting us (subject to legal expectations, such as where disclosures are required by law).

As required by the Public Hospitals Act (PHA) and the Personal Health Information Protection Act (PHIPA), the Health Records Department protects the privacy and confidentiality of your record and retains your record for the allotted time period that is outlined in PHIPA.

You may access and correct your personal health records or request a copy of your chart for legal or other purposes by contacting the health records department at 613-256-2500 extension 2258. Please note that a fee will be charged to cover administrative costs. 

Carleton Place & District Memorial Hospital

You may access and correct your personal health records or request a copy of your chart for legal or other purposes by contacting the health records department at 613-257-2200 extension 813. Please note that a fee will be charged to cover administrative costs.
