Part of being a community hospital is knowing your “community”. This is accomplished by taking the time to listen to what is important to people and their loved ones. At Almonte General Hospital, Carleton Place & District Memorial Hospital, Fairview Manor and the Lanark County Paramedic Service we want to hear from our patients. 

While at the hospital, patients will be invited to participate in an online patient experience survey. Patients will be given a QR code on discharge to voluntarily complete a short survey.  Thanks for your input!

Likewise, Fairview Manor residents and family members are invited to complete the Resident Satisfaction and Family Satisfaction surveys which are distributed once per year. This information is used to plan services and continually improve the home-like provided to our residents.

The Lanark County Paramedic Service also invites patients to complete a satisfaction survey which is provided to patients after transport.
